Carmen Moore

Carmen Moore, MS, RN, BSN, CLS is currently a Manager, Community Nursing with Parkview. Ms. Moore started with Parkview Health in 1981 after graduating from Parkview School of Nursing. She as worked in multiple areas of Parkview Health for many years, several years with Park Center Community Mental Health Center as well as Lincoln National Life as a Behavioral Health Specialist.

Currently employed with Parkview Health, she is the Manager of Parkview Community Nursing. Ms. Moore is responsible for the growth and development of the Community Nursing Initiatives under the direction of Parkview Community Health Improvement. She has built partnerships with multiple external agencies to support outreach programs improving access to services and community awareness of health disparities, barriers and other issues.

Ms. Moore has built and maintained a network of health services to the under-connected, uninsured and underinsured populations in Allen County.

Under Ms. Moore’s leadership, Parkview Community Nursing staff provide educational classes, home visits, and many other community resources to populations in Fort Wayne.

Ms. Moore earned a Bachelor of Science in Nursing degree from Indiana Wesleyan University and earned a Master of Integrated Healthcare Management at Western Governors University. She has been an active member of Chi Eta Phi Professional Minority Nurse Sorority since 1999 and has served as a board member for AIDS Taskforce (now named The Positive Resource Center), Fort Wayne Community Schools Quality Improvement team member, The Fort Wayne Women’s Bureau and the United Way Youth Investment committee and a community mentor for at risk female youth. She is an active member of Faith Missionary Baptist Church.
